Continuum Therapy, Inc. provides outpatient home-based occupational therapy for adults with intellectual, developmental, and long-term physical disabilities.

Environmental safety assessment and modification recommendations, adaptive strategies, and safety-focused interventions to improve mobility, function, and reduce risk.

Developing daily routines and structure, building life skills such as meal preparation, budgeting, and time management, and supporting social participation and community reintegration.

Sensory-Based Strategies

Identifying sensory responses across domains such as touch, movement, sound, sight, taste, and proprioception, with development of a personalized structured plan of sensory activities and environmental modifications to help regulate the nervous system.

Developing strategies to manage chronic health conditions and barriers in order to maintain function, improve quality of life, and work toward meaningful goals.

Supporting Goals Beyond the Clinic

These services allow a client to work with a licensed occupational therapy practitioner and other appropriate providers to meet their goals through multiple avenues.

Training

Daily Living Skills Training

We provide training and education to assist a client in developing and mastering skills necessary for increasing independence with daily activities, both at home and within the community.

Caregiver Training

We provide caregiver education and practical training to support safe, consistent care in everyday routines. Training is fully customized based on the needs of the agency or caregiver team.

Consultation and Education

We provide services to clients and caregivers to improve client independence and safety within the community by improving caregivers' ability to carry out therapeutic interventions and related recommendations. This is often for clients who may struggle with chronic conditions (spasticity, balance impairment, weakness, constipation, dehydration, weight loss or gain, skin breakdown and pressure management through positioning, and pain management).

  • Assessment of needs for management of acute or chronic conditions, often including gathering information and collaboration with other medical and healthcare providers
  • Developing support and intervention plans for management of acute or chronic conditions
  • Training and assistance to carry out the plans
  • Monitoring of client and caregiver during implementation of plans
